Minister Butler rewrites the playbook with unconventional Chief of Staff appointment

In a move that has taken many by surprise, Federal Minister for Health, Aged Care and Disability, Mark Butler, has appointed a new Chief of Staff, Dr Lucas de Toca PSM, who is a radical departure from convention.

While Australia has seen a medical doctor serve as a Health Minister – Dr Michael Wooldridge being a notable example – it has never seen a clinician step into the most powerful staffer position in the Health Minister’s office. That will change in September.

Dr de Toca’s credentials are hard to rival. A senior public servant in the Department of Foreign Affairs and Trade (DFAT), he has led the Global Health Division and the Indo-Pacific Centre for Health Security. His remit included managing Australia’s relationships with multilateral health organisations and overseeing international development assistance in health, a role that placed him at the centre of global vaccine diplomacy during the COVID-19 pandemic.

During the pandemic, he headed the Primary Care Response at the Department of Health, overseeing the rollout of COVID-19 vaccines across primary care sites, coordinating 150 GP-led Respiratory Clinics, and leading infection control initiatives. His focus on Indigenous and remote communities reflected a longstanding commitment to addressing systemic health inequities.

Prior to COVID, Dr de Toca was responsible for child and family health and stakeholder engagement across the Indigenous health sector. He first joined the Department of Health as Principal Advisor, where he oversaw the national taskforce tackling the syphilis outbreak in northern and central Australia, alongside other Indigenous health initiatives.

This appointment signals a deliberate recalibration by Minister Butler – a political leader who has taken charge of approximately $220 billion in annual government spending, constituting more than a quarter of the federal budget. As someone with leadership ambitions and an eye on legacy, Minister Butler spent more time on the election campaign trail alongside Prime Minister Anthony Albanese than any other minister.

Dr de Toca’s arrival marks the end of an era shaped by current Chief of Staff Nick Martin, defined by political acumen and stakeholder management. What Minister Butler appears to be seeking now is a trusted partner capable of overseeing a vast reform agenda, holding the bureaucracy to account, handling the sharp edges of stakeholder engagement across industry, hospitals and insurers, and navigating the growing complexity of global policy shifts arising from the Trump Administration.

By appointing a clinician with frontline, policy, and diplomatic credentials rather than a traditional political strategist, Minister Butler is signalling a shift in how influence is wielded inside the health portfolio. It reflects a growing recognition that the challenges facing healthcare require more than political muscle.
